WAIVER AND AGREEMENT:

I have read the enclosed information, and understand and agree to participate in the tournament. I agree that the tournament reserves the right to use my likeness and voice in any way, without renumeration paid to me for my performances and appearances. I agree to hold Dbuyer, Inc., Elvis & More Entertainment, and 2300 Arena harmless for any and all injury tha may occur to me or my property during my participation in the event, even if the result of such injury is a result of negligence. Upon signing this form, I agree to obey the rules of this tournament and will complete an additional waiver/participation form upon arrival to the tournament.
